FY2025 CMS Service & Spending Variation Index data for 6,642 scored hospices, organized by state and by measure.

Status: proposed, not final. These scores come from the SSVI file CMS published with the FY2027 Hospice Wage Index proposed rule (CMS-1851-P, April 2026). CMS is explicit that the SSVI is not a finding of fraud, waste, or abuse — it flags claims patterns that may warrant additional oversight.

CMS scored every Medicare hospice 0–16 on the SSVI: a Non-Hospice Spending Score (0–8) plus a Utilization Score (0–8) built from eight claims-based measures. Higher is worse. Each state page below shows the 10 highest FY2025 scores in that state (names masked — CCNs shown, so you can look any hospice up), what those hospices have in common, and where to start fixing each measure.

Rankings use the FY2025 total SSVI score from the CMS file (ties broken by FY2024 score). Hospices CMS marked with insufficient data are excluded. We mask hospice names on these pages — the point is the patterns, not the pillory — but every row shows the CCN, and any score can be verified in our free SSVI lookup tool.
