The Service and Spending Variation Index (SSVI) is a hospice scoring system CMS introduced in the FY2027 Hospice Wage Index proposed rule (CMS-1851-P). For the first time, CMS published a provider-level score for nearly every U.S. hospice, built entirely from claims data, to add transparency around hospice utilization and non-hospice spending. A higher score signals greater variation from CMS's expected range — and a potentially higher level of program-integrity concern.
The total SSVI runs from 0 to 16 and is the sum of two parts:
Lower is better. A score near 0 means the hospice sits close to CMS's target range on both spending and utilization. A score near 16 means it is among the highest outliers. Because the score is relative, the lookup above also tells you what percentage of scored hospices you rank better than, and your raw percentile. In FY2025 CMS scored 6,642 hospices, and in FY2024 it scored 6,735.
A dash (–) in the CMS file means CMS did not have enough data to calculate a score for that hospice in that fiscal year. It is not a zero. A hospice may be scored in one fiscal year and not the other.
No. CMS is explicit that the SSVI is not a direct indicator of fraud, waste, or abuse. A high score represents a potentially higher level of concern that may warrant increased transparency and oversight — not a determination of wrongdoing.
